抗苗勒氏管激素(AMH)

AMH在雌性生殖时期由卵巢的颗粒细胞产生,并在生长分化和卵泡形成中起关键作用。具体而言,AMH的表达可抑制原始卵泡的募集并降低卵泡对FSH依赖性选择的敏感性。除了其在卵巢中的功能作用外,血清AMH水平还可作为卵巢储备的生物标志物。

总体而言,30-44岁正常健康女性的AMH水平较高,与自然受孕的自然生育能力呈正相关。AMH还有助于评估多囊卵巢综合征和卵巢早衰等情况。

诊断

AMH是二聚体糖蛋白分子,由通过硫桥连接的两个相同亚基组成。每个亚基含有前区(pro-AMH或N-末端)和C-末端结构域(也称为“成熟”区域),其在两个结构域之间的一元位点处被切割。切割后,前区(110-kDa)和C-末端(25kDa)同二聚体保持与非共价复合物结合,所述非共价复合物与AMH受体II结合以激活信号传导。

测量血清或血浆中AMH水平的诊断测试通常是定量夹心ELISA,其使用针对稳定的前区和成熟区中的表位的抗体。
