CMV
Human cytomegalovirus (CMV, also called Human herpesvirus is a member of the herpes virus family and shares the characteristic ability to remain latent within the body for life within an infected individual. Although a CMV infection is typically asymptomatic in healthy persons, immunocompromised individuals such as AIDS patients, organ transplant recipients, and newborn infants, are at high risk of developing life-threatening complications from primary infections and reactivations. Most people (85%) have been infected by CMV by 40 years of age.

巨细胞病毒 (CMV)
巨细胞病毒不被认为是具有高度传染性的病毒。它通过与体液,如尿液,唾液,乳汁,移植器官和输血直接接 触而传播。健康孕妇不常见巨细胞病毒感染,在怀孕期 间第一次感染病毒的风险约为5-8%。然而,一旦感染 则可能会导致新生儿严重的并发症。在巨细胞病毒感染 的新生儿(先天性巨细胞病毒感染)中,约20%的新生儿会出现终身残疾。到目前为止,还没有可用来防止巨 细胞病毒的疫苗。公共卫生措施的重点是减少巨细胞病 毒感染孕妇,育龄妇女,以及其他高风险人群。
先天性巨细胞病毒感染
先天性巨细胞病毒感染是指新生儿在出生前感染该病毒而出现的一组症状,这是全世界的先天性病毒感染的最常见原因。只有10%的先天性巨细胞病毒感染的新生儿在出生时显示异常,而80% - 90%婴儿会在出生后头几年出现 巨细胞病毒感染并发症。先天性巨细胞病毒的并发症包括听力下降,视力障碍和不同程度的智力低下。巨细胞病毒对胎儿造成损害的风险几乎完全与妇女在怀孕期间的原发性巨细胞病毒感染有直接关系。胎儿先天性巨细胞病毒的 并发症发生的风险似乎对于至少在受孕6个月前感染病毒的孕妇较小。
诊断
已经开发出的用于检测巨细胞病毒感染的各种诊断测试包括病毒培养,血清学检测,PCR分析和细胞病理学。其中, 使用巨细胞病毒 pp65抗原的单克隆抗体检测在白细胞中巨细胞病毒主要的基质蛋白(pp65)的pp65抗原试验在临 床使用历史最长。然而它的主要缺点包括阳性结果判定的主观性,耗时且复杂的程序,难以标准化,并且需要大量的白细胞。IgG抗体/ IgM抗体的酶联免疫吸附试验是检测巨细胞病毒的IgM抗体,IgG抗体和IgG抗体亲和力,它已成为最常用的血清学检测。巨细胞病毒IgM抗体的检测阳性表示急性或原发性病毒感染,而IgG抗体检测阳性表示过去曾经被感染。在IgM抗体和IgG抗体两个均为阳性的情况下,IgG抗体亲和力的水平可以帮助鉴别是急性感染还是陈旧 的感染。出于个原因,新型的检测方法已经把测定抗巨细胞病毒 IgG抗体的亲和力指数与抗巨细胞病毒的IgM抗体的 检测结合在一起了。
为了提高巨细胞病毒抗体检测的灵敏度和特异性,在过去的二十年中, 超过15种在自然感染情况下,具有免疫原性的 巨细胞病毒蛋白的结构多肽已经被研究出来,它们的结构已经被确定。选择抗原的组合是影响检测试机的灵敏度和特异性的最关键要素。最佳的蛋白是:
• CMV pp150: 在病毒的潜伏和再激活期均可被检出的被膜磷酸蛋白。在原发病毒感染期间针对pp150的抗体可能出 现较晚。
- CMV pp52: 病毒的主要的DNA结合蛋白,非结构的核磷酸蛋白。它被看作是血清学转化的早期标志物。
- CMV pp65: 构成病毒主要结构的磷酸蛋白(组成低级矩阵)和病毒颗粒外结构的主要成分。针对它的抗体应答是检 测病毒早期感染的唯一标志。
- CMV gb Antigen: 病毒的膜糖蛋白,它是病毒包膜的最丰富的成分也是中和抗体的靶点。
- CCMV pp38: 病毒的结构蛋白,有证据表明是在病毒早期感染时出现的重要免疫蛋白。
研究还表明,针对巨细胞病毒结构蛋白(pp150和PP38)的IgM抗体检测对于巨细胞病毒重复感染的早期诊断十分关键。多个体外诊断试剂制造商对此已经采用巨细胞病毒裂解物和巨细胞病毒重组蛋白质组合使用以提高检测试剂性能。。
